01 — Board Certified Family Physician
Dr. Afshan Hameeduddin
MD
Dr. Hameeduddin is a board certified family physician with over 25 years of vast clinical experience. She is on staff at St. Alexius Medical Center, Alexian Brothers Medical Center, and Northwest Community Hospital. She takes care of complicated critically ill patients and excels in maintaining great communication with patients and their families. Her outpatient experience includes providing primary care to patients of all ages. She specializes in chronic illnesses management and preventive care to help patients avoid medical issues before they begin.
Dr. Hameeduddin has a great passion for geriatric patients. She is on staff at reputed area nursing homes and assisted facilities. She is Medical Director and Consultant for Clearbrook programs and Medical Director at Bella Terra Schaumburg Skilled Nursing Facility. She is proud to be a family physician providing the best medical care for you and your family.

02 — Board Certified Family Medicine Physician
Dr. Reena Paul
MD
Dr. Reena Paul is a board certified family medicine physician and excited to be serving the Hoffman Estates area in her new office. She is grateful for her established patients that have chosen to follow her to her new location. She looks forward to spending the rest of her career in her home town and establish new long-term patient-doctor relationships.
Dr. Reena Paul completed her residency at Cook County-Loyola University-Provident Family Practice Residency Program in Chicago, Illinois. Dr. Paul later completed a fellowship in faculty development.
She is on medical staff at St. Alexius Medical Center and her interests include adult, geriatric, and preventive care. She is devoted to comprehensive health care for people of all ages. Her interests are disease prevention and health promotion and she is active in community outreach programs including health fairs. Dr. Paul is currently an Associate Professor for Loyola University Medical Center and plays an active faculty role for the Cook County-Loyola University-Provident Family Medicine Residency Program teaching medical students from the University of Illinois, Rush University and Loyola University. Dr. Paul strives to give excellent medical care to her patients and is currently an active member of the American Academy of Family Medicine and Society of Teachers in Family Medicine.

03 — Physician Assistant
Naveed Hyderi
PA
Naveed graduated in 2000 from Nova Southeastern University in Fort Lauderdale, Florida. She was raised in the USA as a child and comes from a very diverse background. Being of Indian ethnicity, she has worked with the underserved and a range of multiple cultures and has a deep understanding of people and values. Naveed is trilingual and communicates fluently in Spanish, Hindi, and Urdu.
Naveed is an outstanding physician assistant with a friendly personality and works very independently in all areas of the clinic. She also sees patients in skilled nursing homes, assisted living facilities, as well as makes home visits.
